I, Bella Forbes authorize my friend Kelly Ted who lives in Boston as an agent or attorney-in-fact of my own child. She is a responsible person and a trustworthy friend. She will take care of my child as an agent for some time. I have talked to her before writing this letter and discussed all the responsibilities that she needs to address.
So now I am officially appointing Kelly Ted as an agent of my blood-related child for five months. After that, I will take all the rights back from her. There is a specific reason that forced me to take this decision for my child’s safety. I am having bad terms with my husband and so I don’t want my kid to be affected by all these things. That’s why I have decided to keep him out of it.
As I have mentioned earlier we had a discussion regarding the responsibilities, I find it important to state all the given rights here in this letter. These responsibilities include that she will take care of my child’s proper diet, she will prefer homemade food, she will be taking responsibility for his school and studies, she will pick and drop him at home by herself, she is allowed to sign the documents related to his educational matters if needed, she will make sure that he will not be going outside alone.
All these duties of my child will be taken over by Kelly from the day after tomorrow till five months.

This is Jamal writing this letter for the authorization of my child. I authorize my mother who lives in the USA, as attorney-in-fact of my child for two years. I believe that she can take care of my daughter better than me these days. I have discussed everything with her related to the responsibilities. She is willing for this duty and I officially in this letter declare my mother as attorney-in-fact of my daughter for two years.
In case, she is not able to perform her duty well or couldn’t pay good attention to my daughter, as she is aged now, I want to give these rights to my aunt who also lives in the USA. She is also single and has no child so I believe that she can also give time to my daughter.
I have certain reasons for doing so. I have to go out for some time related to my job which demands the visit of different places. And also, I am a single parent so it would be difficult for me to take good care or give time to my daughter. This forced me to take this decision and hand over my child to some good hands.
Moreover, the responsibilities include her good diet, she also needs to take care of her medicines as she is a heart patient, she needs to call her driver to pick and drop her home on time, she will also be dealing with her educational matters and allowed to attend meetings and sign documents if needed.
All these responsibilities should be addressed by an agent carefully. As soon as I will return, take these rights back from them.
